Youth Camp

The Oregon Lightning Youth Initiative Football and Cheer Camps are designed to give young athletes in Madras, Prineville, Redmond, and Bend the opportunity to learn, grow, and build confidence through sports. These camps provide hands-on instruction in football and cheerleading, while also teaching teamwork, discipline, leadership, and a strong sense of community.

Funds raised help cover camp operations, equipment, facility use, training materials, staffing, and other resources needed to create a safe, positive, and memorable experience for every participant. Our goal is to make these camps accessible, impactful, and inspiring for youth across Central Oregon.

Through the Oregon Lightning Youth Initiative, we are investing in local kids by creating opportunities for skill development, encouragement, mentorship, and community connection both on and off the field.

I understand that participation in youth football, cheerleading, and related camp activities involves inherent risks, including but not limited to falls, collisions, physical contact, stunts, jumps, tumbling, drills, heat-related illness, dehydration, sprains, fractures, concussion, serious injury, permanent disability, and in rare cases, death.

I understand that The Oregon Lightning Youth Initiative camp may involve running, agility drills, football instruction, non-contact or contact drills, cheer motions, jumps, tumbling, conditioning, and other physical activities associated with youth sports training.

I certify that my child is physically able to participate unless otherwise disclosed on this form.

I voluntarily give permission for my child to participate in this camp.

Liability Release

To the fullest extent permitted by law, I release and hold harmless The Oregon Lightning Youth Initiative, The Oregon Lightning, their staff, coaches, volunteers, sponsors, partners, facility providers, and representatives from any claims, demands, damages, injuries, losses, or expenses arising from my child’s participation in camp activities, except where such release is not permitted by law.

Emergency Medical Authorization

I authorize The Oregon Lightning Youth Initiative and its representatives to obtain emergency medical treatment for my child if I cannot be reached immediately.

I understand that I am financially responsible for any medical care provided to my child.

Concussion and Injury Policy

I understand that if my child is suspected of suffering a concussion or other serious injury during camp, they may be removed from participation and may not return unless appropriate clearance is provided.

Media Release

I, as the parent or legal guardian of the participant listed on this form, hereby grant permission to The Oregon Lightning Youth Initiative and The Oregon Lightning to use photographs, video recordings, and other media containing my child’s image, likeness, or participation taken during camp activities for promotional, educational, fundraising, advertising, website, print, and social media purposes. I understand that no compensation will be provided for such use.
