open tryouts

I acknowledge that it is my responsibility to carry personal health insurance. I understand that Oregon Lightning does not provide medical insurance coverage for participants and will not be held responsible for any costs related to injuries sustained during the tryout.

I understand that football is a high-contact sport that carries a risk of injury. By participating in this tryout, I voluntarily assume all risks associated with participation, including but not limited to injury, illness, or accident. I release and hold harmless Oregon Lightning, its coaches, staff, and affiliates from any and all liability for injuries or damages that may occur during or as a result of the tryout.

In the event of an injury or medical emergency during the tryout, I authorize the staff of Oregon Lightning to seek medical treatment on my behalf if I am unable to do so. I understand that I am responsible for any and all medical expenses incurred as a result of such treatment.

I grant Oregon Lightning and Sullins Photo permission to use my name, image, and likeness in any photographs, video recordings, or media taken during the tryout for promotional and marketing purposes, without compensation.
